What this instrument is for:
The tool can be administered to dialysis patients every six months to evaluate
their self-report of quality of life and to identify patients who are at
risk for stopping dialysis because of a poor rating. The questionnaire also
identifies areas in pain and symptom management and psychosocial and spiritual
support in which interventions could be used to improve patients' quality
of life.
